Comfort Keepers 799 provides Group Health Care Coverage options to eligible full-time employees The plan year for Comfort Keepers 799 is January 1, 2023 through December 31, 2023. Eligible employees have the opportunity to elect to enroll in coverage after they have met their enrollment waiting period and eligibility for coverage. This year, for the 2022 Plan Year, open enrollment begins on November 1st through November 14th. Please be sure to return your completed elections, enrollment, or waiver of coverage no later than November 14, 2022 in order to be covered for the 2023 Plan Year.

The Medical coverage is changing from United Health Care to Blue Care Network, effective January 1, 2023.
The Voluntary Dental coverage will remain through Beam Benefits, policy number MI02183 . Beam Benefits uses the DenteMax Dental Network for participating providers. The Voluntary Vision coverage will be through Beam as well using the Vision Network VSP. To learn more if your provider participates, click on the provider directory link(s) below.
How to complete your Open Enrollment
Eligible employees are strongly encouraged to go through the above provided benefit materials and seek answers to questions regarding your available health care coverage. Comfort Keepers 799 Human Resources and our dedicated agent (AJM Associates, Inc.) can help assist. Below you will find detailed contact information, should you have questions while reviewing your options. Once you have selected the coverage(s) you would like to enroll in, please return to Human Resources or you may email them directly to our agent Karl J. Ruth Jr. at email@example.com, the completed 2023 Benefits Election Form. In addition return a completed carrier enrollment form for your elections. Should you wish to fax them to our agent, that fax number is (586) 591-5931.
All enrolling employees will need to complete the Blue Care Network HMO Enrollment Form in addition to completing the 2023 Comfort Keepers Election Form. Employees can use the above Provider Directory link to search for their participating provider as well as locate their providers NPI to be included on the Blue Care Network HMO Enrollment Form. Employees that do not designate a participating Primary Care Physician of their choosing will be auto assigned a Primary Care Physician by Blue Care Network.
Since the medical coverage is new with Blue Care Network this year, all eligible employees electing to Waive Medical coverage, effective January 1, 2023 will need to complete the Blue Care Network Waiver Form
If you are enrolling in the Beam Voluntary Dental or Voluntary Vision plan for the first time – return a completed Beam Enrollment Form. If you are continuing your existing coverage with no changes, the only form needing to be returned is the completed 2023 Election Form.
If you are not making any changes to your current benefit elections for 2023 or waiving coverage for 2023 on all benefits available, employees will still need to complete the 2023 Benefit Election Form as well as the Blue Care Network Waiver Form and return the completed 2023 forms to Human Resources for your personnel files.

Employees that work on average 30 hours or more per week during the calendar year are considered full-time employees and are eligible for the Employer Sponsored Medical Health Care Plan through Blue Care Network Comfort Keepers 799 will keep the same flat weekly deduction single premium for medical enrollment at a rate of $42.75 per week as it was last plan year. Those employees that wish to enroll their eligible spouse or dependents will be responsible for the additional premium.
All eligible employees may also enroll in the Voluntary Dental and Voluntary Vision coverage through Beam. These coverage's are paid for 100% by employee payroll weekly deductions.
